Surgical hip dislocation is a process that may be applied during orthopedic surgery in case of hip impingement or any other condition that can cause hip pain. It is important not to confuse it with a dislocated hip in which a trauma or congenital condition results in an unintentional dislocation.
Most patients that have a hip injury come to India to have their treatment here in India as the country is known to have the best orthopedic surgeons and have all the required up-to-date equipment. Not only that the treatment cost in India is far lower compared to other countries.
What is Hip Dislocation?
Hip dislocation also referred to as traumatic hip dislocation is a condition whereby the head of the thighbone known as the femur is moved out of the socket in the hip bone, which is referred to as the pelvis. One often requires a force to move the hip joint, and this normally only happens through the use of force. Most dislocations are a result of car accidents or falls from great heights, and thus, usually accompanied by other injuries such as of the bones. Hip dislocation is a severe medical situation that requires immediate medical intervention. Immediate treatment is necessary.
How Is It Caused?
Traumatic hip dislocations are most often caused by a motor vehicle collision. Dislocation can be found frequently when the knee strikes the dashboard of the vehicle in an accident. This force causes the thigh to go backward thereby causing the ball head of the femur to move out of the hip socket. It is more important to wear seatbelts since these could reduce the chances of hip dislocation during an accident by a huge margin.
An injury such as a car or a fall, especially when one falls from a big height like a ladder or any other form of accident in an industry can also lead to this kind of hip dislocation. Though much rarer than shoulder dislocations, any type of collision in a sport such as football or hockey can force the hip joint out of place.
In the case of hip dislocations which can be either anterior or posterior, other associated injuries include, fractures to the pelvis & legs, and back, abdominal, knee & head injuries. Another common type may be the one where the head of the femur comes into contact with the back part of the hip socket and fractures. This kind of injury is known as a posterior wall acetabular fracture-dislocation.

How Is It Diagnosed And Treated?
If there is no other injury apart from the hip dislocation, the orthopedic surgeon can diagnose the condition merely by observing the leg position. Hip dislocations are rarely isolated therefore your doctor will undertake further assessment to determine the extent of the injury. To decide whether a surgical dislocation is necessary, the surgeon has to follow the patient’s symptoms and then compare radiographic findings (X-rays, MRI, CT) and a physical examination.
Procedure
In the course of the operation, the surgeon cuts and peels off a small segment of the bone called the greater trochanter —which is situated at the upper end of the femur and forms a junction with theervation for muscles that encircle the hip and enables its mobility.
This type of surgery is referred to as a greater trochanteric osteotomy. This ensures the surgeon is in a position to dislocate the hip effectively without compromising the blood vessels within the joint and around retaining necessary circulation and muscles. The surgeon is then able to visualize fully and treat the abnormalities of the bones and soft tissues surrounding the hip joint.
Often, standard labral repairs or reshaping can be done, as well as treatment of impingement from the acetabulum or upper femur. Abnormalities that had been caused by previous childhood ailments like SCFE or Perthes can be enhanced. To ensure that the new surgical site is well stabilized during healing, screws are used to hold the separated trochanteric bone fragment and surrounding muscles in position before suturing the incision.

Recovery time
Dislocation of the hip that requires surgical repair also results in a reported hospital stay of between one to three days and a crutch use of at least four to six weeks.
Physical therapy is initiated when the patient is still in the hospital; this includes instructions on the use of a walking aid, recommendation of limited protected weight bearing as well as restriction of motion of particular joints during the recovery process, and muscle strengthening exercises. Outpatient physical therapy begins at six weeks when the bone is healing. Patients are followed up periodically over the first year of surgery to evaluate whether there is adequate healing.
